A new meta-analysis published in JCEM, 2015 confirms the safety advantage of transdermal vs oral estrogens with respect to the risk of venous thromboembolism (VTE). However, significant differences emerged in interpreting data. Based on “the observational nature of the studies and inconsistency of results”  the quality of evidence was described as very low by the authors. We believe that this statement is not appropriate. The heterogeneity of data reported in this meta-analysis is mainly due to the progestogens but not to the route of estrogen administration. We therefore proposed to rate up the quality of evidence supporting the differential effect of oral vs transdermal estrogens on VTE (JCEM, 2016).

In any case, based on this quantitative assessment, The Endocrine Society made a strong recommendation for a non-oral route of estrogens administration among women with increased VTE risk (JCEM, 2015).
